<p>This book describes Compartment Syndrome, Diagnosis and Treatment and Related Diseases<br /> Compartment syndrome is a serious medical disorder that causes higher pressure in a muscle compartment.<br /> It can result in muscle and nerve damage and disorders with blood flow.<br /> Compartment syndrome is produced by a higher pressure within a closed anatomical space, which involves the circulation and function of the tissues within that space.<br /> This may lead to temporary or permanent injury to muscles and nerves.<br /> Compartment syndrome can be acute or chronic:<br /> Acute compartment syndrome is most often produced by trauma, which may be relatively minor.<br /> Chronic compartment syndrome is normally produced by exercise and manifests with recurrent pain and disability, which stop when the cause is ceased but return when the activity is restarted.<br /> Causes<br /> Thick layers of tissue, called fascia, separate the groups of muscles in the arms and legs from each other.<br /> Inside each sheet of fascia is a restricted space called a compartment.<br /> The compartment affects the muscle tissue, nerves, and blood vessels.<br /> Fascia encloses these structures, similar to the way in which insulation envelops wires.<br /> Any swelling in a compartment will lead to higher pressure in that area.<br /> This higher pressure compresses the muscles, blood vessels, and nerves.<br /> If this pressure is increased too much, blood flow to the compartment will be obstructed.<br /> This can cause permanent damage to the muscle and nerves.<br /> If the pressure lasts long enough, the muscles may die and the arm or leg will not move.<br /> Acute compartment syndrome may be caused by:</p> <ol> <li>Trauma, such as a crush injury or surgery</li> <li>Broken bone</li> <li>Very bruised muscle</li> <li>Severe sprain</li> <li>A cast or bandage that is too tight</li> <li>Fractures</li> <li>Crush injury.<br /> Symptoms</li> <li>Decreased sensation, numbness, tingling, weakness of the affected area</li> <li>Paleness of skin</li> <li>Serious pain that does not go away after taking pain medicine</li> <li>Swelling or inability to move the affected part<br /> Acute compartment syndromes normally manifest within 48 hours of injury.<br /> Diagnosis<br /> Diagnosis is basically medical with identification of patients at danger and early signs.<br /> Intra-compartmental pressure is measured by several methods, such as needle manometry, infusion methods, wick catheter, pressure transducers or side-ported needles<br /> MRI scans help make the diagnosis of compartment syndrome in medically uncertain cases</li> </ol> <p>Treatment<br /> The purpose of treatment is to avoid permanent injury.<br /> For acute compartment syndrome, surgery is required immediately.<br /> Any delay in surgery can result in permanent injury.<br /> The surgery is called fasciotomy and requires cutting the fascia and muscle to alleviate pressure.<br /> All possibly constricting dressings, casts and splints must be taken off.<br /> Splitting a plaster is not adequate.<br /> The compartment pressure should be gauged.</p> <p>Open fasciotomy:<br /> Indications for fasciotomy differ between different doctors such as:</p> <ol> <li>An absolute compartment pressure greater than 30-40 mm Hg with a medical picture consistent with compartment syndrome;</li> <li>Difference between diastolic pressure and compartment pressure less than 30 mm Hg or</li> <li>Difference between mean arterial pressure and compartment pressure less than 40 mm Hg.<br /> The skin and deep fascia must be opened out along the whole length of the compartment.<br /> All four compartments may require to be opened in cases affecting the leg.<br /> Following fasciotomy, the wound should be kept open.<br /> Recovery may be helped by suturing, skin grafting or the wound left to recover by itself.<br /> Debridement may be required for any muscle necrosis<br /> The affected area must be elevated above heart level to reduce swelling</li> </ol> <p>TABLE OF CONTENT<br /> Introduction<br /> Chapter 1 Compartment Syndrome<br /> Chapter 2 Causes<br /> Chapter 3 Symptoms<br /> Chapter 4 Diagnosis<br /> Chapter 5 Treatment<br /> Chapter 6 Prognosis<br /> Chapter 7 Paresthesia<br /> Chapter 8 Carpal Tunnel Syndrome<br /> Epilogue</p>画面が切り替わりますので、しばらくお待ち下さい。 ※ご購入は、楽天kobo商品ページからお願いします。※切り替わらない場合は、こちら をクリックして下さい。 ※このページからは注文できません。

The aim of <em>Cell Death Signaling in Cancer Biology and Treatment</em> is to describe state-of-the-art approaches and future opportunities for achieving this goal by targeting mechanisms and pathways that regulate cancer cell death. In this book, molecular defects in cell death signaling that characterize cancer cells, including dysregulation of cell death due to overexpression/hyperactivation of oncoproteins, as well as the loss of tumor suppressor proteins will be described. <p>Agranulocytosis</p> <p>Agranulocytosis is a dangerous condition which can occur from exposure to chemotherapy.</p> <p>One of my patients with lung cancer had just died from severe infection following the loss of white blood cells following her chemotherapy.</p> <p>What is Agranulocytosis?</p> <p>Agranulocytosis is a medical disorder in which the bone marrow does not make enough of a certain form of mature white blood cells (neutrophils).</p> <p>The bone marrow is the blood producing tissue inside bones.</p> <p>Agranulocytosis is a reduction in circulating neutrophils in the blood which forms 4-5% of total body neutrophil stores</p> <p>Most of the neutrophils are stored in the bone marrow, either as mitotically active (one third) or post-mitotic mature cells (two thirds)</p> <p>Granulocytopenia is defined as a reduced number of blood granulocytes such as neutrophils, eosinophils, and basophils.</p> <p>Agranulocytosis is frequently used synonymously with neutropenia and is therefore confined to the neutrophil lineage alone.</p> <p>The danger of serious infection rises as the absolute neutrophil count (ANC) drops to the severely neutropenic range (< 500/μL).</p> <p>The period and severity of neutropenia is directly linked with the total incidence of all infections and of those infections that can threaten life.</p> <p>Causes</p> <p>Agranulocytosis may be due to:</p> <ol> <li>Autoimmune disorders</li> <li>Bone marrow diseases, such as myelodysplasia or large granular lymphocyte (LGL) leukemia</li> <li>Certain medicines used to treat diseases</li> <li>Chemotherapy</li> <li>Certain street addiction drugs</li> <li>Poor nutrition</li> <li>Preparation for bone marrow transplant</li> <li>Genetic problems</li> </ol> <p>Agranulocytosis results in a person not having enough of a specific form of white blood cells, such as neutrophils or granulocytes.</p> <p>A low neutrophil count may also happen when white blood cells are destroyed faster than they can be produced.</p> <p>Symptoms</p> <p>People with this disorder tend to have fevers and infections.</p> <p>Frequent presenting symptoms of neutropenia are:</p> <ol> <li>Low-grade fever</li> <li>Sore mouth</li> <li>Odynophagia</li> <li>Gingival pain and swelling</li> <li>Skin abscesses</li> <li>Recurrent sinusitis and otitis</li> <li>Symptoms of pneumonia (e.g., cough, dyspnea)</li> <li>Perirectal pain and irritation</li> </ol> <p>Patients with agranulocytosis normally present with:</p> <ol> <li>Sudden onset of malaise</li> <li>Sudden onset of fever, possibly with chills and prostration</li> <li>Stomatitis and periodontitis together with pain</li> <li>Pharyngitis, with difficulty in swallowing</li> </ol> <p>Lung infections are normally bacterial or fungal pneumonias.</p> <p>Physical signs on evaluation of a patient with agranulocytosis are:</p> <ol> <li>Fever</li> <li>Stomatitis</li> <li>Periodontal infection</li> <li>Cervical lymphadenopathy</li> <li>Skin infection: The skin examination focuses on rashes, ulcers, or abscesses</li> <li>Splenomegaly</li> <li>Related petechial bleeding</li> <li>Perirectal infection</li> <li>Growth retardation in children</li> </ol> <p>In agranulocytosis, the following may be present:</p> <ol> <li> <p>Fever (often 40°C or higher)</p> </li> <li> <p>Rapid pulse and respiration</p> </li> <li> <p>Hypotension and signs of septic shock if infection has been present</p> </li> <li> <p>Painful aphthous ulcers in the oral cavity</p> </li> <li> <p>Swollen and tender gums</p> </li> </ol> <p>Diagnosis</p> <p>The diagnosis of agranulocytosis requires the exclusion of infectious and drug-induced causes of neutropenia through the following laboratory tests:</p> <ol> <li> <p>Full blood count: Including a differential count in assessing patients of agranulocytosis</p> </li> <li> <p>Differential white blood cell count</p> </li> <li> <p>Peripheral smear review by a pathologist</p> </li> </ol> <p>The following studies apply to some patients with neutropenia:</p> <ol> <li> <p>Antinuclear antibody</p> </li> <li> <p>Rheumatoid factor</p> </li> <li> <p>Serum immunoglobulin studies</p> </li> <li> <p>Liver function tests</p> </li> <li> <p>Peripheral blood flow cytometry</p> </li> <li> <p>T-cell gene rearrangement for T-cell clonality</p> </li> <li> <p>Paroxysmal nocturnal hemoglobinuria testing:</p> </li> </ol> <p>By high-sensitivity or fluorescent aerolysin (FLAER)?based flow cytometry</p> <ol start="8"> <li>Antineutrophil antibodies:</li> </ol> <p>Tne marrow aspiration or a biopsy from the posterior iliac crest should be obtained.</p>画面が切り替わりますので、しばらくお待ち下さい。 ※ご購入は、楽天kobo商品ページからお願いします。※切り替わらない場合は、こちら をクリックして下さい。 ※このページからは注文できません。

<p>Our insight into the mechanisms of the physiology of reproduction has experienced a swift and constant development these last few years. The advent of more sophisticated diagnostic methods and their relatively easy clinical application allow for the incorporation of that knowledge into the evaluation of the infertile couple. These facts, together with an obvious change in social psychology, have facilitated the development of different specialities dealing with the problems of infertility. It is now possible in medical centers all around the world to undertake a better disposition of the infertile husband to look for advice, to be studied, and treated. Confronted with this situation we are still unable to offer specific therapy in most cases; however, empirically based therapy abounds. Advances in therapy have not kept pace with our increased physio logical knowledge and improved diagnostic techniques. Patho physiological mechanisms and etiological factors in male infertility are largely unknown. This has significantly hampered both clinical evaluation and advances in treatment allowing for frequent non scientific therapeutic incursions into the armamentarium of the an drologist. Several factors have given birth to therapeutic "fashions", which are bound to survive as long as this state of lack of knowledge con tinues. For example, we may ask whether the treatment of varicocele constitutes a fashion? <p><strong>Treatment-resistant Depress</strong></p> <p>Successful management of patients with treatment-resistant depression requires a thorough understanding of the biological basis for both the depression and its failure to respond to standard treatments. Chapter 3 describes several genes already associated with treatment-resistant depression and, while further work is needed to translate findings into clinical recommendations, suggests that genetic prediction of treatment resistance could become a widespread clinical reality within a few years.</p> <p>Most patients with treatment-resistant depression will be treated pharmacologically, so three chapters review the latest evidence for pharmacological best practice in switching strategies for antidepressants, the role of antipsychotics and augmentation strategies to complement lithium.</p> <p>There are two major alternatives to pharmacotherapy: neuromodulation and psychotherapy. The brain intervention chapter summarizes clinical research and experience with electroconvulsive therapy, transcranial magnetic stimulation, vagus nerve stimulation, deep brain stimulation and magnetic seizure therapy. The final chapter reviews the literature pertaining to the effectiveness of various forms of psychotherapy in patients who have not responded to antidepressant pharmacotherapy, explaining that patients who have not responded to one or two trials of antidepressant medication have a 30%-50% chance of responding to a focused psychotherapy.
