<p>Bacterial diseases are infectious in nature, i.e. they can be transmitted from one host to another. Outbreaks of infectious diseases are often compared with forest fires. Once a fire has spread through an area, it does not return until new trees have grown up. Outbreaks develop when a large population of susceptible animals is present. lf immunized animals are present in the herd, then outbreaks cannot occur until a sufficient number of young, non-resistant animals have been bred or non-immune animals from outside are introduced in the herd. The development of a bacterial disease is a complex process involving many factors. Many a time, control activities reflect compromises among alternatives. To proceed intelligently, one must identify components of the bacterial dissemination which are primarily responsible for a particular disease. Control measures should be directed towards that part of the cycle which is most susceptible to control the weakest links in the chain of disease process. The restriction of movement of animals suffering from, or exposed to, infection is one of the oldest tools known to preventive medicine. The principle of quarantine of domesticated animals was applied as early as roman times. The early detection of a disease in a population of animals- a herd of cattle, for example-is particularly useful in controlling certain chronic infectious diseases, such as mastitis, brucellosis and tuberculosis. Laboratory tests like agglutination test in pullorum disease, the tuberculin skin test for tuberculosis and chemical tests performed on milk to diagnose bovine mastitis are used for the detection of diseases in an animal population. Laboratory tests to confirm the existence of diseased animals in a population, followed by slaughter of the affected animals, have been of great value in controlling infectious and genetic diseases. Bovine tuberculosis has been eliminated from Denmark, Finland and Netherland and reduced to a low level in various other countries like Great Britain, Japan, United States and Canada by this method. <p>This book describes Mallory Weiss Syndrome, Diagnosis and Treatment and Related Diseases</p> <p>Mallory-Weiss syndrome (MWS) is one of the frequent causes of acute upper gastrointestinal (GI) bleeding, featured by the presence of longitudinal superficial mucosal lacerations (Mallory-Weiss tears).</p> <p>Serious and prolonged vomiting can cause tears in the lining of the esophagus.</p> <p>Doctors Mallory and Weiss in 1929 accurately depicted this disorder as lower esophageal lacerations happening to patients with repetitive forceful retching and vomiting after excessive alcohol intake.</p> <p>The esophagus is the tube that joins the throat to the stomach.</p> <p>Mallory-Weiss syndrome (MWS) is a disorder marked by a tear in the mucous membrane, or inner lining, where the esophagus meets the stomach.</p> <p>Most tears recover within 7 to 10 days without treatment, but Mallory-Weiss tears can produce considerable bleeding.</p> <p>Depending on the severity of the tear, surgery may be essential to correct the injury.</p> <p>These tears happen primarily at the gastroesophageal junction and may extend proximally to involve the lower or even mid esophagus and at times extend distally to involve the proximal portion of the stomach.</p> <p>The diagnosis of MWS is normally confirmed with endoscopy.</p> <p>The average esophageal MWS tear is about 2-4 cm in length</p> <p>Most patients have only one tear of the mucosa near the GE junction.</p> <p>The tear is just below the GE junction on the lesser curvature of the stomach.</p> <p>The most frequent cause of MWS is severe or prolonged vomiting.</p> <p>While this type of vomiting can happen with stomach illness, it also often happens due to chronic alcohol abuse or bulimia.</p> <p>Heavy alcohol consumption is regarded as one of the most significant predisposing factors since about 50% to 70% of the patients diagnosed with Mallory-Weiss syndrome have a history of alcoholism.</p> <p>The severity of the upper GI bleeding with Mallory-Weiss syndrome is also documented to be higher with the concurrent presence of portal hypertension and esophageal varices.</p> <p>The relationship between a hiatal hernia and Mallory-Weiss syndrome is still debatable.</p> <p>A hiatal hernia was present in a large number of cases with Mallory-Weiss syndrome while a case-control study found no difference in the incidence of a hiatal hernia between patients with Mallory-Weiss syndrome and the control group.</p> <p>Other risk factors are:</p> <ol> <li>Bulimia nervosa,</li> <li>Hyperemesis gravidarum and</li> <li>Gastroesophageal reflux disease (GERD).</li> </ol> <p>In a large number of patients (around 25% of cases), none of these risk factors were identified.</p> <p>The disorder is triggered by repeated acts of a sudden rise of the intra-abdominal pressure such as retching, vomiting, straining, coughing, cardiopulmonary resuscitation or blunt abdominal traumas.</p> <p>Iatrogenic Mallory-Weiss syndrome is normally infrequent.</p> <p>Mallory-Weiss syndrome does not always cause symptoms in mild cases.</p> <p>This is more frequent in mild instances when tears of the esophagus produce only a small quantity of bleeding and resolve quickly without treatment.</p> <p>In 85% of cases, the presenting symptom is hematemesis.</p> <p>The quantity of blood is inconsistent varying from blood-streaked mucus to massive bright red bleeding.</p> <p>In case of serious bleeding, other symptoms such as melena, dizziness, or syncope can be present.</p> <p>Upper GI endoscopy is the gold standard for specifically diagnosing Mallory Weiss tears, and treating simple active esophageal bleeding.</p> <p>Other treatments are:</p> <ol> <li>Multipolar electrocoagulation (MPEC),</li> <li>Injection of a sclerosant agent,</li> <li>Argon plasma coagulation (APC), or</li> <li>Endoscopic band ligation.</li> <li>Epinephrine local injection</li> </ol> <p>Surgery is seldom required.</p> <p>TABLE OF CONTENT<br /> Introduction<br /> Chapter 1 Mallory Weiss Syndrome<br /> Chapter 2 Causes<br /> Chapter 3 Symptoms<br /> Chapter 4 Diagnosis<br /> Chapter 5 Treatment<br /> Chapter 6 Prognosis<br /> Chapter 7 Gastrointestinal Bleeding<br /> Chapter 8 Esophagitis<br /> Epilogue</p>画面が切り替わりますので、しばらくお待ち下さい。 ※ご購入は、楽天kobo商品ページからお願いします。※切り替わらない場合は、こちら をクリックして下さい。 ※このページからは注文できません。

<p>Cough in children may occur from causes anywhere along the airway, from the nose to the alveoli.<br /> Cough is a non-specific reaction to irritation anywhere from the pharynx to the lungs.<br /> Childhood coughing is a frequent disorder that can cause anxiety in parents.<br /> There are significant differences from adult cough in terms of possible causes and treatment guidelines.<br /> Chronic cough in children is described as a cough that last longer than eight weeks.<br /> This time period is used because most acute infective causes of cough will recover in 3-4 weeks, and the eight-week definition identifies those who may require more investigations.<br /> Occasionally post infective cough may persist longer (3-8 weeks) as in the case of pertussis or post viral cough.<br /> Young children may get repeat occurrences of viral respiratory tract infection up to 6-8 times per year and this may be mistaken for chronic cough.<br /> These guidelines notice that the time period between acute and chronic cough (3-8 weeks) is sometimes called 'sub-acute cough' or 'prolonged acute cough' (e.g., a slowly resolving post-viral cough).<br /> The three most frequent causes of chronic cough in adults (asthma, postnasal drip syndrome and gastro-esophageal reflux) are not always the most frequent in children<br /> 1.Most children with asthma have inflamed or swollen airways, which often cause repeat occurrences of episodes of cough, wheezing and shortness of breath<br /> 2.Postnasal drip produced by rhinitis or a sinus infection (sinusitis) can induce chronic cough.<br /> Normally other symptoms such as nasal blockage and congestion or sneezing are present, but occasionally the only symptom observed is the cough.<br /> 3. In some children, the reason of chronic cough is stomach fluid moving back up the throat or reflux.<br /> Some children may form a hoarse voice and choking as symptoms as a result od acid irritating the throat<br /> 4. Following a viral respiratory infection, healthy children can have a cough lasting for weeks.<br /> 5. <p>Addison Disease is a disease that is caused by the deficiency of circulating glucocorticosteroids and mineral corticoids (aldosterone) hormones as a result of disease of the adrenal glands.<br /> The causes of Addison Disease are:</p> <ol> <li>Auto-immune disease (Type 1 PGA ? <p>Transverse myelitis is a neurological disorder caused by inflammation across both sides of one level or segment of the spinal cord.<br /> Attacks of inflammation can damage or destroy myelin, the fatty insulating substance that covers nerve cell fibers.<br /> This damage causes nervous system scars that interrupt communications between the nerves in the spinal cord and the rest of the body.<br /> Most patients will have only one episode of transverse myelitis; a small percentage may have a recurrence.<br /> The segment of the spinal cord at which the damage occurs determines which parts of the body are affected.<br /> a. Nerves in the cervical (neck) region control signals to the neck, arms, hands, and muscles of breathing (the diaphragm).<br /> b. Nerves in the thoracic (upper back) region relay signals to the torso and some parts of the arms.<br /> c. Nerves at the lumbar (mid-back) level control signals to the hips and legs.<br /> d.
