【電子書籍なら、スマホ・パソコンの無料アプリで今すぐ読める!】A Simple Guide to Thyroiditis, Diagnosis, Treatment and Related Disorders【電子書籍】[ Kenneth Kee ]

This book describes Thyroiditis, Diagnosis and Treatment and Related Diseases

The term thyroiditis indicates inflammation of the thyroid gland.

Thyroiditis can be classified based on the start of symptoms, underlying cause and medical symptoms.

The most frequent cause of thyroiditis is an autoimmune disease.

Thyroiditis can cause temporary or permanent hypothyroidism and hyperthyroidism

Thyrotoxic phase

Thyrotoxicosis indicates that the thyroid is inflamed and secretes too many hormones.

Hypothyroid phase

After the excessive secretion of thyroid hormones for a few weeks or months, the thyroid will not have sufficient thyroid hormones to release.

This causes a lack of thyroid hormones or hypothyroidism.

Euthyroid phase

During the final euthyroid phase, the thyroid hormone levels return to normal.

This phase may come transiently after the thyrotoxic phase before continuing to the hypothyroid phase, or it may terminate after the thyroid gland has healed from the inflammation and is able to maintain a normal hormone level.

Types of thyroiditis

Hashimoto’s thyroiditis: an autoimmune disorder produced by anti-thyroid antibodies.

This is the most frequent type of thyroiditis and 5 times more frequent in women than in men.

Hashimoto’s thyroiditis normally leads to hypothyroidism, and thyroid hormone replacement treatment is required.

Silent thyroiditis or painless thyroiditis: another autoimmune disease produced by anti-thyroid antibodies.

It is also frequent in women and the next frequent cause after Hashimoto’s thyroiditis.

Post-partum thyroiditis: an autoimmune disorder produced by anti-thyroid antibodies that occasionally happens shortly after a woman gives birth

Radiation-induced thyroiditis: a disorder produced by external radiation used as a medical treatment for certain cancers, or by radioactive iodine used to treat hyperthyroidism

Sub-acute thyroiditis or de Quervain’s thyroiditis: an often painful disorder believed to be produced by a virus

Acute thyroiditis or suppurative thyroiditis: a relatively rare disorder produced by an infectious organism or bacterium

Drug-induced thyroiditis: a disorder is produced by the use of drugs such as amiodarone, interferons, lithium, and cytokines.

Thyroiditis can happen due to:
Autoimmune disease (Hashimoto thyroiditis, Graves disease, postpartum thyroiditis or painless sporadic thyroiditis),
Infection (painful sub-acute thyroiditis or suppurative thyroiditis),
Drugs (amiodarone, lithium, interferons and interleukin-2; and checkpoint inhibitors) or
Fibrosis (Riedel thyroiditis)

It can be categorized into acute, sub-acute, and chronic thyroiditis.

Acute thyroiditis is produced by bacterial infection of the gland.

Sub-acute, also termed granulomatous thyroiditis, is produced by viruses.

Chronic involves autoimmune thyroiditis, Hashimoto thyroiditis, postpartum thyroiditis, and other painless thyroiditis, such as iatrogenic and drug-induced thyroiditis.

Thyroid-stimulating hormone (TSH) should be examined to see if the patient has hyperthyroidism or hypothyroidism.

Thyroid microsomal antibodies and thyroid receptor antibodies can be examined for confirmation of autoimmune thyroid disease.

Erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) are raised in infectious thyroiditis (bacterial).

Thyroid ultrasound is the most important and most often used imaging method for evaluation of the thyroid gland.

The treatment of thyroiditis is dependent on the type, symptoms, and phase of thyroiditis.

TABLE OF CONTENT
Introduction
Chapter 1 Thyroiditis
Chapter 2 Causes
Chapter 3 Symptoms
Chapter 4 Diagnosis
Chapter 5 Treatment
Chapter 6 Prognosis
Chapter 7 Hashimoto’s Thyroiditis
Chapter 8 Myxedema
Epilogue

Recently, research efforts aiming to improve energy efficiency of wastewater treatment processes for large centralized wastewater treatment plants (WWTPs) have been increasing. Global warming impacts, energy sustainability, and biosolids generation are among several key drivers towards the establishment of energy-efficient WWTPs. WWTPs have been recognized as major contributors of greenhouse gas emissions as these are significant energy consumers in the industrialized world. The quantity of biosolids or excess waste activated sludge produced by WWTP will increase in the future due to population growth and this pose environmental concerns and solid waste disposal issues. Due to limited capacity of landfill sites, more stringent environmental legislation, and air pollution from incineration sites, there is a need to rethink the conventional way of dealing with wastewater and the sludge production that comes with it. This book provides an overview of advanced biological, physical and chemical treatment with the aim of reducing the volume of sewage sludge.

Provides a comprehensive list of processes aiming at reducing the volume of sewage sludge and increasing biogas production from waste activated sludge.

Includes clear process flowsheet showing how the process is modified compared to the conventional waste activated sludge process.

Provides current technologies applied on full scale plant as well as methods still under investigation at laboratory scale.

Offers data from pilot scale experience of these processes

The human hair being an ornament which it is almost everybody's endeavour to preserve inviolate as long as possible, it is explicable, why the fear of losing that ornament has become a matter of speculation of charlatanism on the credulity of the public. The best means of confounding the designs of such speculations, is the statement of real facts, distributing thus the light of knowledge.

The following pages are intended to be a popular treatise, aiming at showing that the laws by which the life of the hair is governed differ not from the laws to which the body at large is subject, and that deviations of normal development must be consideredーas is the case in other organsーas disease and treated as such.

This book intensively examines the efficacy of plant-derived products that have been used for over a thousand years by practitioners of so-called Traditional Chinese Medicine in the light of recent chemotherapeuticals. The chapters were written by renowned Chinese medical researchers and are supplemented by results obtained in German antiparasitic research projects.

Parasites and emerging diseases are a major threat of our time, which is characterized by an enormous increase in the size of the human population and by an unbelievably rapid globalization that has led to the daily transport of millions of humans and containers with goods from one end of the earth to the other. Furthermore the slow but constant global warming offers new opportunities for many agents of diseases to become established in new areas. Therefore it is essential that we develop precautions in order to avoid epidemics or even pandemics in overcrowded megacities or at the large-scale farm animal confinements that are needed to secure a steady flow of food in the crowded regions of the world.

Of course intensive research in the field of chemotherapy since 1900 has produced unbelievable breakthroughs in therapies for formerly untreatable and thus deadly diseases. However, a large number of untreatable diseases remain, as well as a constantly growing number of agents of disease that have developed resistances to standard chemical compounds.

As such, it is not only worthwhile but also vital to consider the enormous amounts of information that have been obtained by human “high cultures”in the past. Examples from the past (like quinine) or present (like artemisinin, a modern antimalarial drug) show that plant extracts may hold tremendous potential in the fight against parasites and/or against vector-transmitted agents of diseases.

Lowe syndrome (oculocerebrorenal syndrome) is characterized by:

Vision disorders such as clouding of the lenses of the eyes (cataracts) that are present at birth,

Kidney disorders that normally develop in the first year of life, and

Brain abnormalities that are linked with intellectual disabilities.
Lowe syndrome is inherited as an X-linked genetic disorder.
Lowe syndrome is a rare genetic disorder that happens almost specially in males.
The frequency is evaluated to be between 1 and 10 males per 1,000,000 people.
It has been identified in most cultures around the world and seems to have the same frequency in all populations.
Because Lowe Syndrome is an X-linked syndrome, the majority of affected persons are males.
There have been a few cases of females diagnosed with Lowe Syndrome.
In these cases, the female typically has one inactive X-chromosome and the active X-chromosome has the mutation for Lowe Syndrome.
Lowe syndrome (the oculocerebrorenal syndrome OCR) is a multiple system disorder that is typically featured by anomalies involving the eye, the nervous system and the kidney.
It is an infrequent, pan-ethnic, X-linked disease.
Bilateral cataract and severe hypo-tonia are present at birth.
In the later weeks or months of the infant, a proximal renal tubulopathy (Fanconi-type) is apparent and the ocular feature may be complicated by glaucoma and cheloids.
Psychomotor retardation is evident in childhood, while behavioral disorders are present and renal complications occur in adolescence.
The mutation of the gene OCRL1 localized at Xq26.1 is accountable for the disease
Phosphatidylinositol 4,5-bisphosphate 5-phosphatase INP51 controls the enzyme phosphatidylinositol (4,5) bisphosphate 5 phosphatase, PtdIns (4,5)P2, in the trans-Golgi network
Both enzymatic and molecular testing is available for confirmation of the diagnosis and for prenatal detection of the disease.
Life span seldom exceeds 40 years.
Mothers may be carriers with

25% possibility of having an affected boy,

25% possibility of having a carrier daughter,

25% possibility of having an unaffected boy,

25% possibility of having a non-carrier girl.
Female carriers of Lowe syndrome may be diagnosed in 94% of patients by slit-lamp examination because of the presence of considerably punctuate white to grey opacities, issued in a radial fashion in all layers of the lenticular cortex.
Using flanking linked markers, DNA analysis can show the same molecular defect (in a heterozygous state) as that previously recognized in the patient.
Boys with Lowe syndrome have cataracts that are present at birth in each eye
About half of eyes will form high pressure in the eye (glaucoma) that can injure the optic nerve
Infants with Lowe syndrome have poor muscle tone (hypo-tonia) at birth.
The kidney disorders linked with Lowe syndrome is called proximal tubular dysfunction of the Fanconi type.
There is no cure for Lowe Syndrome, but many of the symptoms can be treated successfully by:

Medicines,

Surgery,

Physical and occupational treatments,

Vision therapy,
The treatments are:

Cataract extraction,

Glaucoma control,

Physical and speech therapy,

Use of drugs to address behavioral problems, and

Correction of the tubular acidosis
Seizure disorders are treated with anticonvulsant medications.

Behavior disorders are treated with behavior modification and medications.
Early intervention programs are advised and should start in early infancy:

Physical therapy,

Occupational therapy,

Speech and language therapy,

Special education services

Services for visually impaired.
The longest reported survival is that of a 54 year-old patient.

TABLE OF CONTENT
Introduction
Chapter 1 Lowe Syndrome
Chapter 2 Causes
Chapter 3 Symptoms
Chapter 4 Diagnosis
Chapter 5 Treatment
Chapter 6 Prognosis
Chapter 7 Cataract
Chapter 8 Renal Failure
Epilogue

Tendonitis is the inflammation and swelling of the enclosed or sheathed tendons.
Tendons are the strong fibrous structures that attach muscles to bones.
When these tendons become swollen or inflamed, it is termed tendonitis.
In many patients, tendinosis (tendon degeneration) is also present.
It can happen together with synovitis (inflammation of the tendons sheaths or synovium).
It can also produce stenosing tendonitis (tightened inflammation of the tendon sheaths).
Causes:
The most frequent cause is excess use of the involved tendon.
These disorders are more frequent in middle-aged adults and mostly in people who are play sports.
They may be more frequent if the work requires repetitive movements such as writing, typing or use of a computer mouse.
Some risk factors are:

Injury or damage to the tendon

Repetitive movements of finger and wrist joints

Arthritis of the joints may be prone to tendonitis

Systemic disorders such as multiple sclerosis, amyloidosis, rheumatoid arthritis

Tendonitis happens in families, and is normally appear more often in males than in females

Tendonitis can happen as a result of injury or excess use.

Playing sports is a frequent cause.

Tendonitis also can happen with aging as the tendon’s elasticity becomes loosen.
Body-wide (systemic) diseases, such as rheumatoid arthritis or diabetes, can also result in tendonitis
The main symptoms are pain, tenderness and occasionally swelling of the affected part of the tendon.

Pain on movement of the tendon
a. Pain and tenderness along a tendon, normally near a joint
b. Pain at night
c. Pain that becomes worse with movement or action

Swelling of part of the tendon affected

Stenosis of the tendon sheath may be present

Stiffness in the morning

Stiffness of the tendon - inability to stretch
Signs:

Swelling over the affected tendon

Passive stretching of the tendon is painful.

There may be local tenderness of the inflamed tendon.

The tendons sheaths are normally swollen and thickened

The tendons may become stuck in the narrowed tendon sheaths (Trigger Finger)

There may be crepitations over the tendon as it travels across the sheaths
There are definite tests for specific tendons.
The tendon may be inflamed, and the skin over it may be warm and red.
If an infection is the suspected cause (rare) then blood tests and other tests may be performed to find the cause of the infection.
Occasionally, if the diagnosis is not certain, the doctor may suggest:

Ultrasound scan

X-ray

MRI
Treatment may be conservative:

Rest of the tendon

Splints or brace

Cold or ice

Pain killers such as NSAID

Muscle relaxant

Physiotherapy
A physiotherapist will give the patient a program of exercises to slowly make the muscles of the involved tendon stronger.
This will require doing exercises that raise the load that the muscle can carry.

Injection of local anesthetic and long acting steroid into the affected sheath or tendon nodule may help to reduce inflammation
Surgery Treatment:
Surgical opening of the synovial sheath may be necessary for tendon release.
In rare cases, surgery is needed to remove the inflamed tissue from around the tendon.
Other treatments are also being given to treat tendonitis:

Shock-wave therapy

Autologous blood injection
Blood is taken from the patient and then injected into the region around the injured tendons.

TABLE OF CONTENT
Introduction
Chapter 1 Inflamed Tendons (Tendonitis)
Chapter 2 Causes
Chapter 3 Symptoms
Chapter 4 Diagnosis
Chapter 5 Treatment
Chapter 6 Prognosis
Chapter 7 Trigger Finger
Chapter 8 De Quarvian’s Disease
Epilogue

Mitral regurgitation is a medical condition in which the heart valve that separates the upper and lower chambers on the left side of the heart does not close properly.
Regurgitation means leaking from a valve that does not close all the way.
Mitral regurgitation is the most common type of heart valve disorder.
As the valve does not close properly, some blood is pumped back into the left atrium when the left ventricle contracts
Therefore the wall of the atrium may become thicker (hypertrophy) and the atrium may enlarge
This leads to a decrease in blood flow to the rest of the body.
As a result, the heart may try to pump harder.
Causes are:

Coronary heart disease and high blood pressure

Infection of the heart valves

Mitral valve prolapse (MVP

Use of fenfluramine or dexfenfluramine for 4 or more months.
When symptoms occur, they often develop gradually, and may include:

Cough

Fatigue, exhaustion, and light-headedness

Rapid breathing

Sensation of feeling the heart beat (palpitations) or a rapid heartbeat
Diagnosis is by:
Extra heart sound (S4 gallop)
Apical soft pan-systolic murmur
Echocardiogram shows the mitral regurgitation
ECG and CXR show a normal heart if mild but left ventricle enlargement in later MR.
Patients with high blood pressure or a weakened heart muscle may be given medications to reduce the strain on the heart and help improve the condition.

Beta-blockers or ACE inhibitors

Blood thinners (anticoagulants) to help prevent blood clots in people with atrial fibrillation

Drugs that help control uneven or abnormal heartbeats

Water pills (diuretics) to remove excess fluid in the lungs

A low-sodium diet may be helpful
Surgical repair or replacement of the valve is recommended if:

The heart function is poor,

The heart becomes larger (dilated)

Symptoms become more severe

TABLE OF CONTENT
Introduction
Chapter 1 Mitral Regurgitation
Chapter 2 More about Mitral Regurgitation
Chapter 3 Treatment of Mitral Regurgitation
Chapter 4 Heart Valve Diseases
Chapter 5 Mitral Stenosis
Chapter 6 Aortic Regurgitation
Chapter 7 Tricuspid Regurgitation
Chapter 8 Pulmonary Regurgitation
Epilogue

Mitral Valve regurgitation is a rare congenital valve disease
Where the mitral valves do not close tightly and may release
Blood may leak from the ventricle back into the atrium
This may cause strain on the left atrial myocardium

Mitral regurgitation is the most common type of heart valve disorder
Blood flows backward into the atrium from the lower chamber
Therefore the wall of the atrium may become thicker
As a result the heart may need to pump harder

The mitral valve makes an apical systolic murmur, a whooshing sound
When the valve leaks back into the left atrium a small amount
The stretched flaps can make an extra sound (S4) as they shut.
The echocardiogram show the valve movement in the heart

Treatment is to reduce the heart's workload by medications
Beta blockers may be used to treat chest discomfort or palpitations
Blood-thinning medicines reduce the risk of blood clots forming
Surgery is needed if the abnormal mitral valve allows the blood backflowing

-An original poem by Kenneth Kee

Translational medicine underpins vascular medicine. It is fundamental to understanding how we treat patients with vascular disease and more importantly, how to prevent it. It is the rationale for drug design and production. Vascular medicine and translational medicine will take over and become the main reason for referring patients to hospital. Therefore, hospital-based clinicians working with basic scientists need to know about translational medicine, which educates and informs them about vascular medicine and how management should be based.

This book is a primer for translational vascular medicine and discusses the evolving and exciting areas of basic science applied to vascular medicine. The book is based on the third vascular biology conference held at The Royal College of Physicians in 2008. It provides a large amount of new basic and clinical information and the contributors are world leaders.

The discussion of some diseases is only occasionally enlivened by the emergence of new worthwhile facts. In the case of others, it would seem that, even if only a few years have passed, each new discussion wears the air of a revolution. Hodgkin's disease, at least from the pathological standpoint, is not so extraordinarily fickle and certainly does not touch either of these two extremes of variability. But it is still a fertile field of study and presents its full share of innovations and practical results. This is perhaps due to the special position it occupies between tumours and inflammatory diseases, or possibly to a lucky series of coincidences; the truth is that events have been on the move for some years now in the case of this disease. There can be no doubt that surprising progress has been made in connec tion with Hodgkin's disease, due, one feels, to close cooperation between several branches of medical science, each of which has had occasion to make new and useful contributions. This does not, however, hide the fact that certain important matters are still not clear, in particular the cause of the disease, its essential nature and, indeed, the best method for its treatment.

This volume is a first of its kind, addressed principally to the professional reader. While it is not intended to be exhaustive, its aim is to sketch a broad picture of some of the nondrug and nonsurgical treatment strategies with a demonstrated basis in conventional scientific method. Likewise, though it does not include all those who have contributed to the emergence of this exciting new field, it assembles those authors whose seminal work has earned them international reputations.

This volume's declared purpose is to provide a state-of-the-art guide to methods and techniques in the behavioral treatment of epilepsy and to their basis in theory. The editors hope that it will catalyze the evolution of their acceptance as standard elements, where appropriate, in the clinical activities of independent practitioners, clinics, and agencies that service those with convulsive disorders.

How the Back Pain Industry Is Costing Us More and Giving Us Lessーand What You Can Do to Inform and Empower Yourself in Seeking Treatment【電子書籍】[ Richard A. Deyo, MD ]

Over the past twenty years, treatment of back pain has become ever more expensive and intensive. Use of MRI scans, narcotic painkillers, injections, and invasive spine surgery have all grown by several hundred percent. In some areas of medicine, newer treatments have improved quality and duration of life, but as back pain is treated more aggressively, annual surveys of people with back pain report steadily worse impairments. In Watch Your Back!, Richard A. Deyo, MD, proposes an approach to managing back pain, which most adults in the United States experience at some point, that empowers the individual and leads more directly to effective care.

Though it may seem counterintuitive, fewer medical interventions may produce better results. Expecting a probe, a pill, or a procedure to cure back pain is usually unrealistic, yet entire industries promote the notion that someone else will "fix" you. Watch Your Back! exposes these flaws in the current approach to back pain, along with the profit motives and conflicts of interest behind many of them. The book dramatizes the problems with stories of prominent individuals who encountered high-tech pitfalls, then found low-tech solutions suited to their lifestyles and the nature of their back pain.

Watch Your Back! will be useful not only for people with back pain but also for doctors and policy makers. Our health care system has a growing interest in reducing waste, overuse, and unnecessary care. There's a consensus that health care is too expensive and that we get too little for the money. Back pain exemplifies a problem for which we can simultaneously improve quality of care and reduce costs.
