<p>The term "urticaria" refers to any of a group of distinct skin conditions that are characterized by itchy, wheal-and-flare skin reactions (hives).</p> <p>In spontaneous urticaria, the most common type, the hives seem to arise without provocation.</p> <p>Spontaneous urticaria is divided into acute (lasting less than six weeks) and chronic types.</p> <p>The pathognomonic itching, hives, and angioedema arise by the same mechanism--cutaneous mast cell activation and release of histamine and other mediators of inflammation--in both acute and chronic urticaria, but these two disorders have different etiological profiles.</p> <p>The underlying cause of acute urticaria cannot be identified in about half of all cases.</p> <p>Chronic urticaria, which is much rarer, is usually caused by autoreactivity, chronic infection, or intolerance to food additives.</p> <p>If the condition persists after the underlying cause has been treated or eliminated, non-sedating antihistamines are the agents of first choice for symptomatic treatment.</p> <p>Unlike acute urticaria, which is self-limited and should be treated symptomatically, chronic urticaria should be treated by the identification and elimination of underlying causes, which is usually curative.</p> <p>An attempt has been made in this Booklet to include the Image Gallery and to discuss comprehensively the types of urticaria, angio-edemas, diagnosis, pathophysiology of urticaria, various laboratory tests and treatment options along with several illustrations.</p> <p>…Dr. H. <p>Pancoast Syndrome or tumors are cancers that form at the very top part of the lung (the apex).<br /> Pancoast tumors are a subset of non-small cell lung cancers that invade the top of the chest.<br /> Because of their location, they invade adjoining tissue<br /> Cancers in the top part of the lung are rare.<br /> Pancoast tumor normally spread into one or more structures in the top part of the chest which are:</p> <ol> <li>The top ribs in the chest (the thoracic ribs)</li> <li>Nerves in the top of the chest</li> <li>Bundles of nerves close to the spinal cord</li> <li>Blood vessels that supply blood to the arms<br /> Most pancoast tumors are a type called non small cell cancer and most often squamous cell cancer.<br /> Causes<br /> The risk factors for almost all lung cancers are similar.</li> <li>Smoking</li> <li>Secondary smoke exposure</li> <li>Prolonged asbestos exposure</li> <li>Exposure to industrial elements (such as gold or nickel)</li> <li>Exhaust fumes from cars<br /> Symptoms<br /> While a Pancoast tumor is a lung tumor, it rarely causes symptoms that are typically related to the lungs (like cough or chest pain).<br /> ecause the cancer is at the top of the lungs, it might put pressure on or damage a group of nerves that runs from the upper chest into the neck and arms.<br /> The group of nerves is called the brachial plexus.<br /> Pressure on the brachial plexus can cause several very specific symptoms:</li> <li>Severe pain in the shoulder or the shoulder blade (scapula)<br /> The initial symptom is normally pain in the shoulder, the inner part of the shoulder blades, or both.</li> <li>Pain in the arm and weakness of the hand on the affected side<br /> The affected person normally needs to support the elbow of the affected arm in the opposite hand to ease the tension on the shoulder and upper arm.</li> <li>Horner's syndrome<br /> There is flushing on one side of the face and that side does not sweat.<br /> The eye on the same side has a smaller (constricted) pupil with a drooping or weak eyelid.</li> <li>In up to 25% of people with a Pancoast tumor, compression of the spinal cord and paralysis of the lower half of the body develop when the tumor extends into the opening between two vertebrae.<br /> Diagnosis<br /> Pancoast tumors can be difficult to diagnose.<br /> There is a need for an MRI scan to help diagnose it.<br /> Arteriogram or venogram with dye may show the lung cancer<br /> Bronchoscopy may help<br /> Biopsy can make a diagnosis in 95%<br /> Mediastinoscopy and PET scan may show spread of cancer<br /> Treatment<br /> Treatment for a pancoast tumor is dependent on the stage of the cancer, its exact position in the lung and the general health<br /> The standard of care for people with a Pancoast tumor is chemotherapy and radiation followed by removal of the tumor and a portion of the chest wall if it is affected or if its removal facilitates surgery.<br /> The reason for the chemotherapy and radiation is to reduce the size of the tumor and to prevent the cancer from spreading through lymph nodes.<br /> An interval of two to four weeks after the chemo and radiation allows the procedures to reach their maximal effect<br /> After four weeks, all patients are reassessed for surgery.<br /> If the cancer has not spread to distant areas of the body, surgery will likely be offered.<br /> Surgery<br /> Before surgery, the doctor carefully assesses and stages the cancer.<br /> During surgery, the doctor normally removes a portion of the chest wall and part of the lung.<br /> If the patient is fit enough to have surgery, he or she normally have a combination of chemotherapy and radiotherapy (chemoradiotherapy) first to shrink the cancer.<br /> This is called tri-modality treatment.<br /> The surgery requires removing the top two ribs or sometimes more.<br /> Immunotherapy and targeted cell cancer therapy have changed the treatment of lung cancer and have improved survival rates tremendously.</li> </ol> <p>TABLE OF CONTENT<br /> Introduction<br /> Chapter 1 Pancoast Syndrome<br /> Chapter 2 Causes<br /> Chapter 3 Symptoms<br /> Chapter 4 Diagnosis<br /> Chapter 5 Treatment<br /> Chapter 6 Prognosis<br /> Chapter 7 Horner Syndrome<br /> Chapter 8 Immunotherapy for Lung Cancer<br /> Epilogue</p>画面が切り替わりますので、しばらくお待ち下さい。 ※ご購入は、楽天kobo商品ページからお願いします。※切り替わらない場合は、こちら をクリックして下さい。 ※このページからは注文できません。

<p>Bursitis is the medical disorder of inflammation or irritation of the bursa.<br /> The bursa is a sac containing lubricating fluid, located between tissues such as bone, muscle, tendons, and skin that reduce rubbing, friction, and irritation.<br /> The bursa is a fluid-filled sac that works as a cushion between a muscles, tendons, and bones.<br /> Occasionally the cause may not be found.<br /> Bursitis often happens in the shoulder, knee, elbow, and hip.<br /> Other regions that may be involved are the Achilles tendon and the foot.<br /> Bursitis is most often produced by repetitive, minor injury on the area, or from a sudden, more serious trauma.</p> <ol> <li>An atypical or poorly placed bone or joint (such as length sizes in the legs or arthritis in a joint) can place added stress on a bursa sac, causing bursitis.</li> <li>Stress or inflammation from other disorders, such as rheumatoid arthritis, gout, psoriatic arthritis, thyroid disorders, or abnormal medicine reactions may also raise a person's risk.</li> <li>Also, an infection can sometimes result in inflammation of a bursa.</li> <li>The most frequent causes of bursitis are repetitive movements or locations that cause irritation of the bursae around a joint:<br /> a. Throwing a baseball or lifting something over the head repeatedly<br /> b. Leaning on the elbows for long periods<br /> c. Extensive kneeling for tasks such as laying carpet or scrubbing floors<br /> d. Prolonged sitting, especially on hard surfaces<br /> The most frequent locations for bursitis are in the shoulder, elbow and hip.<br /> But the patient can also have bursitis by the knee, heel and the base of the big toe.<br /> Symptoms of bursitis may be:</li> <li>Joint pain and tenderness when the patient pushes around the joint</li> <li>Stiffness and aching when the patient moves the affected joint</li> <li>Swelling, warmth or redness over the joint<br /> Doctors can often diagnose bursitis based on a medical history and physical exam.<br /> X-ray images cannot positively confirm the diagnosis of bursitis, but they can assist ruling out other causes of the discomfort.<br /> Ultrasound or MRI may be done if the bursitis cannot easily be diagnosed by a physical exam alone.<br /> Magnetic resonance imaging (MRI) may be done to exclude tissue injury, such as a torn tendon (tendons are the cords that join bones to muscles)<br /> Bursitis treatment normally requires conservative measures, such as rest, ice and taking a pain reliever such as aspirin, paracetamol and ibuprofen.</li> <li>Rest the joint until the symptoms get better and prevent any strenuous activities that tend to cause more pain, such as running.</li> <li>Wearing padding may help prevent the joint from more injury.<br /> Knee pads may help if the patient has bursitis in the knee joints.</li> <li>Ice packs are also a good method of reducing inflammation and pain.</li> <li>When sleeping, do not sleep on the side that has bursitis.<br /> If conservative treatments do not work, further treatment may be:</li> <li>Medicine<br /> Non-steroidal anti-inflammatory drugs (NSAIDs), such as ibuprofen can relieve pain<br /> If the inflammation in the bursa is produced by an infection, the doctor might prescribe an antibiotic.</li> <li>Physical Therapy<br /> The doctor may advise physical therapy or exercises to strengthen the muscles in the involved area to ease pain and prevent recurrence.</li> <li>Injections<br /> The doctor may inject a corticosteroid medicine into the bursa to alleviate inflammation in the shoulder or hip.<br /> This treatment normally brings rapid pain relief</li> <li>Assistive device<br /> Transient use of a walking cane or other device will help alleviate pressure on the affected area.</li> <li>Aspiration (Removing fluid from the bursa)<br /> If the swelling produced by bursitis is especially severe, the patient may want to have the fluid drained out.</li> <li>Surgery<br /> a. <p>Herniated Disk or Prolapsed Inter-vertebral Disk is the medical disorder of prolapse of the inter-vertebral disk (which is the disk between 2 vertebrae) as a result of protrusion of the nucleus pulposus of the disk out of its weakened ligamentous ring (annulus fibrosus).<br /> The disk may protrude in a posterior or posterolateral direction producing pressure to the nerve roots particularly at L4, L5 and S1 vertebrae.<br /> Many doctors use the phrase “prolapsed disk” synonymously with terms such as “herniated disk,” “slipped disk,” and “ruptured disk.”<br /> While this is fine for common usage, a prolapsed disk medically refers to the second of four stages of disk rupture.<br /> Along the route to fully rupturing, the disks in the spine may pass through four distinct stages.</p> <ol> <li>Disk Degeneration<br /> During this stage, the outer walls of the inter-vertebral disks are weakened, but no visible external alterations have occurred.</li> <li>Disk Prolapse<br /> The outer wall of the disk begins to change shape and bulge out into the spinal canal.<br /> The patient may begin to feel symptoms at this stage if the distended disk exerts pressure on the spinal cord or the nerve roots that extend from it.</li> <li>Disk Extrusion<br /> At this stage, the gel-like inner core of the disks has burst through a tear in the disk’s outer wall, but has not leaked out into the spinal canal.</li> <li>Disk Sequestration<br /> At this final stage, the gel-like substance has left the disk and has leaked into the spinal canal.<br /> It is almost certain that the patient will feel symptoms at this stage, as the inner gel contains proteins that will inflame nerves that they come into contact with.</li> <li>The most common symptom is Low Backache with pain in the lumbar region.<br /> Pain may so bad that the person cannot stand erect.</li> <li>There is a Sciatica or pain shooting down one leg<br /> Physical Examination in a patient with suspected lumbar (lower back) inter-vertebral disk disease may feature the following:<br /> a.
