<p>Frostbite is the condition of injury to the skin and underlying tissues caused by extreme cold.<br /> Frostbite is the most frequent freezing injury.<br /> Frostbite is an injury that is induced by exposure of parts of the body to temperatures below freezing point.<br /> The cold produces freezing of the skin and underlying tissues.<br /> The fingers, toes and feet are most often involved but other extremities, such as the nose, ears and cheeks, can also form frostbite.<br /> Causes<br /> Frostbite happens when the skin and body tissues are exposed to cold temperature for a long period of time.<br /> The patient tends more likely to develop frostbite if he or she:</p> <ol> <li>Takes medicines called beta-blockers</li> <li>Has poor blood supply to the legs</li> <li>Smoke</li> <li>Has diabetes</li> <li>Has Raynaud syndrome<br /> Normally the blood transports oxygen to all parts of the body so that the body tissues are maintained healthy.<br /> During a protective response when the body has exposure to extreme cold, the blood vessels narrow (constrict) so that blood (and oxygen) are transferred away from the extremities to the vital organs to keep the body alive.<br /> After a while, this inadequate blood supply and oxygen to the skin can begin to produce injury to the cells.<br /> In the parts of the body involved by frostbite, the ice crystals form and cells and blood vessels become injured.<br /> Blood clots can also develop in small blood vessels which will decrease the possibility of blood and oxygen getting to the affected tissues.<br /> The probability of frostbite is raised when the patient has longer exposure to the cold temperatures.<br /> Risk factors<br /> Frostbite is not frequent.<br /> Frostbite most often involves these groups of people:</li> <li>Soldiers.</li> <li>People who work outdoors in the cold.</li> <li>Homeless people.</li> <li>Winter outdoor sports people such as skiers and climbers.</li> <li>Alcohol may make the drunk less obvious to the cold and hence develop frostbite<br /> The different degrees of frostbite</li> <li>First-degree frostbite just involves the epidermis.</li> <li>Second-degree frostbite may involve the epidermis and part of the dermis.</li> <li>Third-degree frostbite involves the epidermis, the dermis and the fatty tissue beneath the dermis.</li> <li>Fourth-degree frostbite occupies the entire thickness of the skin, the tissues that are below the skin, and also the deeper tissues such as muscles, tendons and bone.<br /> Symptoms of frostbite may be:</li> <li>Pins and needles feeling, followed by numbness</li> <li>Hard, pale, and cold skin that has been exposed to the cold for too long</li> <li>Aching, throbbing or lack of feeling in the affected area</li> <li>Red and extremely painful skin and muscle as the area thaws<br /> Very severe frostbite may cause:</li> <li>Blisters</li> <li>Gangrene (blackened, dead tissue)</li> <li>Damage to tendons, muscles, nerves, and bone<br /> Diagnosis:<br /> Frostbite is normally diagnosed by the normal features in someone who has been exposed to extreme, cold weather.<br /> Scintigraphy and Bone Scans are occasionally used to evaluate the degree of frostbite and to determine how much tissue is injured<br /> Treatment<br /> The basic first aid for frostbite injuries is:</li> <li>Shelter the person from the cold and move him or her to a warmer place.</li> <li>Change wet clothing for dry clothing.</li> <li>Keep the involved area air dry and wrapped</li> <li>Try to make sure that the person is re-hydrated.<br /> Give warm water to drink<br /> Rapid re-warming can be done using heated water which should be kept at 37-39°C for 30 minutes<br /> Re-warming can be very painful, hence anti-inflammatory painkillers are also given<br /> Some 'clot-busting' medicines (thrombolytics) may be given in cases of deep frostbite to attempt to improve the blood supply to the involved area.<br /> Surgery is required to remove dead tissue -debridement of injured tissue and amputation of dead fingers or toes</li> </ol> <p>TABLE OF CONTENT<br /> Introduction<br /> Chapter 1 Frostbite<br /> Chapter 2 Causes<br /> Chapter 3 Symptoms<br /> Chapter 4 Diagnosis<br /> Chapter 5 Treatment<br /> Chapter 6 Prognosis<br /> Chapter 7 Cold Injury<br /> Chapter 8 Hypothermia<br /> Epilogue</p>画面が切り替わりますので、しばらくお待ち下さい。 ※ご購入は、楽天kobo商品ページからお願いします。※切り替わらない場合は、こちら をクリックして下さい。 ※このページからは注文できません。

<p>Williams syndrome is an infrequent disorder that can result in physical and mental development.<br /> It is inherited (passed down in families).<br /> Most people with Williams Syndrome are as friendly, happy and pure as the fairies found in the books of myths and tales.<br /> The word "elfin" may have used to describe the facial features of people with Williams syndrome.<br /> People believed that persons with the syndrome, who have extremely charming and kind personalities, had extraordinary, even magical, powers.<br /> They look like the elves with their mischievous upturned noses, pointed chins and flashy smiles.<br /> For these persons with distinct elf-like facial features, the goodness of heart and the vulnerability of trust are symptoms of a genetic disorder (Williams Syndrome) that affects an estimated 1 in 7,500 to 10,000 people.<br /> In spite of their intellectual difficulties, they are chatty, witty and tremendously friendly and they tend to hug everyone they meet.<br /> The syndrome is characterized by distinct facies, connective tissue anomalies, intellectual disability, a specific cognitive profile, unique personality features, cardiovascular disease, growth abnormalities and endocrine abnormalities<br /> Williams syndrome is caused by micro-deletion (not having a copy) of several genes.<br /> Parents may not have any family history of the disorder.<br /> People with Williams syndrome have a 50% chance of passing the disorder on to each of their children.<br /> It often happens randomly.<br /> One of the 28 missing genes is the gene that produces elastin, a protein that permits blood vessels and other tissues in the body to stretch.<br /> It is probable that missing a copy of this gene leads to the narrowing of blood vessels, stretchy skin, and flexible joints observed in this disorder.<br /> Medical presentations of infantile hyper-calcemia are present from birth through to adulthood<br /> The WS phenotype is variable that no single medical feature is needed to establish the diagnosis.<br /> It should be suspected in patients with these findings:</p> <ol> <li>Cardiovascular disease (elastin arteriopathy):<br /> a. Supra-valvular aortic stenosis (SVAS) is the most frequent and medically significant disorder occurring in 75% of patients.<br /> b. Peripheral pulmonic stenosis (PPS) is frequent in infancy even though any artery can be affected.</li> <li>Distinct facies:<br /> a. Broad forehead, bi-temporal narrowing, periorbital fullness.<br /> b. Star-like iris pattern, strabismus.<br /> c. Short nose with broad nasal tip.<br /> d. <p>Dysphagia is defined as difficulty in swallowing.<br /> It is usually associated either with pharyngeal or esophageal disease<br /> There is a spectrum of possible etiologies from self-limiting illness e.g. tonsillitis to carcinoma.<br /> It may occur with odynophagia - painful swallowing.<br /> It occurs in 1-2/100,000, most commonly seen in mid-adult life and is caused by impaired neural control of the distal esophagus.<br /> Several conditions can cause dysphagia.<br /> In children it is often due to:</p> <ol> <li>Physical malformations,</li> <li>Conditions such as cerebral palsy or muscular dystrophy</li> <li>Gastroesophageal reflux disease (GERD).<br /> Dysphagia in adults may be due to tumors (benign or cancerous), conditions that cause the esophagus to narrow, neuromuscular conditions, stroke, or GERD.<br /> It can also be caused when the muscle in the esophagus does not relax enough to let food pass into the stomach.<br /> Other risk factors include smoking, excessive alcohol use, certain medications and teeth or dentures in poor condition.<br /> A. Obstructive causes:</li> <li>Gastro-oesophageal reflux ± stricture.</li> <li>Other esophagitis (e.g. infection).</li> <li>Esophageal cancer.</li> <li>Gastric cancer.</li> <li>Pharyngeal cancer.</li> <li>Post-cricoid web (Paterson Brown-Kelly syndrome).</li> <li>Esophageal rings.</li> <li>Foreign body (acute).<br /> B. Neurological Causes</li> <li>Cerebrovascular event or brain injury.</li> <li>Achalasia.</li> <li>Diffuse esophageal spasm.</li> <li>Syringomyelia or bulbar palsy.</li> <li>Myasthenia gravis.</li> <li>Multiple sclerosis.</li> <li>Motor neurone disease.</li> <li>Myopathy (dermatomyositis, myotonic dystrophy).</li> <li>Parkinson's disease and other degenerative disorders.</li> <li>Chagas' disease.<br /> Steady worsening of dysphagia over a few weeks in an older patient suggests malignancy.<br /> Men with new onset of alarm symptoms (loss of weight with worsening dysphagia) have an increased likelihood of a diagnosis of cancer, especially in those aged over 65<br /> A positive predictive value of 9.0% has been found in this age group.<br /> The most common lesions within the esophagus are inflammatory strictures from reflux or tumors.<br /> In esophageal manometry, a tube is inserted into the stomach to measure pressure differences in various regions.<br /> X-rays of the neck, chest, or abdomen may be taken.<br /> Barium swallow and endoscopy with biopsy should be performed.<br /> In a barium x-ray, moving picture or video x-rays are taken of the esophagus as you swallow barium, which is visible on an x-ray.<br /> MRI scanning may also be required before any surgery is considered, e.g. if there is esophageal carcinoma.<br /> Videofluoroscopy is the radiological investigation of choice when 'difficulty swallowing' rather than 'food sticking' is the presenting symptom and aspiration is suspected.<br /> Esophageal motility studies (require swallowing a catheter containing a pressure transducer) are useful when esophageal spasm is suspected.<br /> The patient may need to chew well or liquidize food.<br /> Definitive treatment depends on cause - usually endoscopic dilation (either using bougies or inflatable balloon).<br /> In esophageal carcinoma, palliative relief of dysphagia can be achieved with repeated dilatation, stent replacement, laser photocoagulation and injection of sclerosants.<br /> Surgical myotomy and endoscopic injection of the sphincter with botulinum toxin is occasionally used</li> </ol> <p>TABLE OF CONTENT<br /> Introduction<br /> Chapter 1 Dysphagia<br /> Chapter 2 Interesting Facts of Dysphagia<br /> Chapter 3 Treatment of Dysphagia<br /> Chapter 4 Stroke<br /> Chapter 5 Parkinson's Disease<br /> Chapter 6 GERD<br /> Chapter 7 Tonsillitis<br /> Epilogue</p>画面が切り替わりますので、しばらくお待ち下さい。 ※ご購入は、楽天kobo商品ページからお願いします。※切り替わらない場合は、こちら をクリックして下さい。 ※このページからは注文できません。

<p>The results of treatment for childhood acute myelogenous leukemia (AML) have improved considerably during the last ten years. This progress was demonstrated by the two consecutive multicenter studies, AML-BFM-78 and -83, in which almost identical extended multi drug regimes of che motherapy were administered for eight weeks and followed up by two years maintenance. The main difference in the second study was the addition of an eight-day intensive in duction course. Due to this new element, the relapse rate was reduced significantly. Another result of the BFM-83 study was the definition of two risk groups on the basis of standardized treatment, which has lead to a risk-adapted treatment strategy in the third ongoing trial, AML-BFM- 87. This improvement was only possible thanks to the coop eration of pediatricians, physicians, radiotherapists, statisti cians, and especially the staff at the hospitals and reference laboratories. Thus, we would like to thank everyone who has been involved in these studies and hope that they will be further encouraged to improve treatment strategies for AML in children. <p>The human prostate gland is of undoubted importance in reproductive physiology and is one of the commonest causes of clinical urological problems in the male patient. Despite the wealth of published lit erature relating to the prostate gland surprisingly little is understood about its neural innervation, the influence of pharmacological factors and its precise physiological functions. Indeed, benign disorders of the prostate gland including benign prostatic hyperplasia, bladder neck dyssynergia and inflammatory disorders of the prostate (prostatitis, prostatodynia) although well recognised clinically, are as yet poorly characterised and understood. It was not until the late 1960s that the subject of urodynamics, which for the first time allowed scientific assessment of the function of the lower urinary tract, was introduced. The first section of this book considers recent advances in our understanding of the innervation of the bladder and prostate gland and the changes in bladder function which accompany prostate medi ated bladder outflow obstruction.
