<p>This book describes Becker’s Nevus, Diagnosis and Treatment and Related Diseases</p> <p>Last week a medical student came to see me for the removal of stitches for a punch biopsy done by his skin specialist in his university in Adelaide, Australia.<br /> He has scattered hyper-pigmented rash through his back and front of chest for the past 4 months.<br /> In spite of treatment the rash had not improved so the skin specialist decided to do a punch biopsy for histological examination under the microscope.<br /> He was informed that the rashes were a form of Becker’s nevus which normally has overgrowth of hair but because he was an asian and devoid of hair on the chest and back, there was no overgrowth of hair on his body.<br /> Treatment is normally by the use of laser to remove the disfiguring rashes.</p> <p>Becker’s Nevus is regarded as a pigmented hamartoma of skin (benign, focal malformation); a hamartoma is a developmental anomaly occurring in childhood.<br /> The term, Becker’s nevus, is actually wrongly named because it is is not a true nevo-melanocytic lesion.<br /> Becker’s Nevus produces:</p> <ol> <li>Abnormal skin discoloration,</li> <li>Abnormal hair growth, and</li> <li>Thickening of skin (verrucous skin surface)<br /> It is a rare disorder that mostly affects males.<br /> Becker’s Nevus occurs during one’s childhood or adolescent stage and the skin lesions are mostly seen on the shoulders and back<br /> Becker’s Nevus may manifest significant complications, such as acne or cosmetic disorders.<br /> Sometimes, melanoma (a type of skin cancer) is known to form from this skin lesion<br /> Becker’s Nevus is known to happen periodically and the cause is largely not known<br /> This disorder is not congenital (not evident at birth).<br /> All racial and ethnic groups are at risk and there is no specific preference observed<br /> Both males and females are involved, though the frequency of Becker’s Nevus is much higher in males compared to females.<br /> The male-to-female ratio derived from documented cases is about 6:1.<br /> Cause<br /> The cause of Becker’s Nevus formation is not known<br /> Genetic mutations (or genetic defects) have been suspected by doctors in some patients, but these have not been well-featured<br /> It is a hamartoma of ectodermal and mesodermal offshoots that is believed to be hormonally reactive given the augmented smooth muscle and peri-pubertal growth.<br /> Androgen receptor assays have shown marked rises in the number of androgen receptors in lesional skin when compared with contra-lateral non-lesional skin.<br /> The increased presence supporting increased sensitivity to androgens is the findings of:</li> <li>Hypertrichosis,</li> <li>Acanthosis,</li> <li>Dermal thickening,</li> <li>Sebaceous hypertrophy and</li> <li>Co-localized acne.<br /> Symptoms:</li> <li>It is a benign disorder that happens as a growth abnormality</li> <li>Abnormally hyper pigmented skin is present.</li> <li>There is also the presence of a thickened skin and unusual hair overgrowth at the site</li> <li>The skin lesion may be painless and non-itchy</li> <li>Becker’s Nevus can happen anywhere on the body.<br /> Diagnosis<br /> A Becker’s Nevus is diagnosed through these tools:</li> <li>Complete medical history with a proper physical examination</li> <li>Dermoscopy: A dermoscope is a diagnostic tool where a dermatologist examines the skin using a special magnified lens</li> <li>Wood’s lamp examination</li> <li>Skin biopsy:<br /> Treatment</li> <li>The doctor may select to regularly observe the benign tumor; a “wait and watch” approach may be followed.<br /> No treatment is normally required</li> <li>If required, a surgical excision (or laser therapy) and complete removal of the lesion</li> <li>If there is Becker’s Nevus linked acne, then treatment therapy for acne may be indicated</li> <li>Laser therapy may also be indicated for hyper-pigmentation and hyper-trichosis</li> </ol> <p>TABLE OF CONTENT<br /> Introduction<br /> Chapter 1 Becker’s Nevus<br /> Chapter 2 Causes<br /> Chapter 3 Symptoms<br /> Chapter 4 Diagnosis<br /> Chapter 5 Treatment<br /> Chapter 6 Prognosis<br /> Chapter 7 Cafe-au-liat Spots<br /> Chapter 8 Melasma<br /> Epilogue</p>画面が切り替わりますので、しばらくお待ち下さい。 ※ご購入は、楽天kobo商品ページからお願いします。※切り替わらない場合は、こちら をクリックして下さい。 ※このページからは注文できません。

<p>This book describes Zollinger-Ellison Syndrome, Diagnosis and Treatment and Related Diseases<br /> Zollinger-Ellison syndrome (ZES) is an endocrine disorder featured by gastrin-secreting tumors, which produce multiple, refractory and repeat peptic ulcers in the distal duodenum and proximal jejunum.<br /> There are two main types:</p> <ol> <li>Sporadic (isolated).</li> <li>ZES linked disorders with parathyroid and pituitary tumors as part of the genetic disorder multiple endocrine neoplasia type 1 (MEN1).<br /> The tumor (gastrinoma) is normally in:</li> <li>The duodenum (60-65%) or</li> <li>The pancreas (30%).<br /> Rarely, gastrinomas happen in other abdominal sites (e.g., the stomach, liver, bile duct, ovary) and also extra-abdominal sites (e.g., the heart, lung - small cell lung cancer).<br /> Causes<br /> Zollinger-Ellison syndrome is produced by tumors.<br /> These tumors are most often present in the head of the pancreas and the upper small intestine.<br /> The tumors are termed gastrinomas<br /> Besides secreting high levels of gastrin, these tumors may secrete other hormones such as:</li> <li>Adrenocorticotrophic hormone (ACTH),</li> <li>Vasoactive intestinal polypeptide (VIP).<br /> They can also form a range of peptides, such as:</li> <li>Insulin,</li> <li>Pancreatic polypeptide,</li> <li>Glucagons,</li> <li>Chromogranin A,</li> <li>Neuron-specific enolase, and</li> <li>The alpha and beta subunits of human chorionic gonadotrophin (hCG).<br /> High levels of gastrin produce formation of too much stomach acid.<br /> Symptoms</li> <li>Abdominal pain</li> <li>Diarrhea</li> <li>Vomiting blood<br /> Signs are ulcers in the stomach and small intestine.<br /> Diagnosis:<br /> It is very frequent for ZES to be treated early as a simple peptic ulcer.<br /> The combination with pain with diarrhea may indicate the diagnosis.<br /> Hyper-gastrinemia is described as fasting serum gastrin concentration >100 pg/ml and is seen often in medical disorders other than ZES such as:</li> <li>Proton pump inhibitor (PPI) or H2-receptor antagonist therapy,</li> <li>Post-vagotomy,</li> <li>Renal failure and</li> <li>Chronic atrophic gastritis.<br /> If ZES is confirmed, it is important to determine MEN1, and calcium, parathyroid hormone and prolactin levels should be evaluated.<br /> If the diagnosis is indicated, then more specific tests are needed - there is one algorithm from Liverpool:<br /> Assessment of fasting gastrin level - levels >1000 pg/ml with acidic gastric juice (pH200 pg/ml 15 minutes after the dose is administered.<br /> Treatment<br /> Medicines called proton pump inhibitors (omeprazole, lansoprazole, and others) are given to decrease acid production by the stomach.<br /> This reduction of acid helps the ulcers in the stomach and small intestine to heal.<br /> In patients with liver metastases, surgery should be done followed by chemotherapy.</li> </ol> <p>TABLE OF CONTENT<br /> Introduction<br /> Chapter 1 Zollinger-Ellison Syndrome<br /> Chapter 2 Causes<br /> Chapter 3 Symptoms<br /> Chapter 4 Diagnosis<br /> Chapter 5 Treatment<br /> Chapter 6 Prognosis<br /> Chapter 7 Barrett Esophagus<br /> Chapter 8 Gastro-esophageal Reflux Disease<br /> Epilogue</p>画面が切り替わりますので、しばらくお待ち下さい。 ※ご購入は、楽天kobo商品ページからお願いします。※切り替わらない場合は、こちら をクリックして下さい。 ※このページからは注文できません。

Internal factors affect the inner structure of the tooth (the dentin) causing it to darken or become yellowish through:</li> <li>Excessive exposure to fluoride during early childhood</li> <li>Tetracycline antibiotics use during the second half of pregnancy and children 8 years old or younger.</li> <li>Tooth injury in a child with damage to the developing permanent tooth.</li> <li>Systemic illness during tooth formation such as hemolytic disease of the newborn</li> <li>Ageing discoloration - the dentin turns yellow naturally with aging making the teeth look discolored.</li> <li>Infection (dental caries) results in grayish discoloration.</li> <li>Dentinogenesis imperfecta<br /> Rare condition in which children are born with gray, amber or purple discolorations in the teeth</li> <li>Amelogenesis imperfecta<br /> Genetic condition in which enamel formed during tooth development is abnormal and causes discoloration of teeth.<br /> Symptoms are:</li> <li>Stains on the enamel such as yellow or brown spots.</li> <li>Yellow stains on the teeth from dentin showing through the worn away enamel.<br /> Diagnosis<br /> A dental examination can diagnose tooth discoloration and some causes (e.g., gingivitis).<br /> Treatment<br /> Treatment depends on the cause of the discoloration and may include.</li> <li>Tooth hygiene -proper tooth brushing and flossing techniques</li> <li>Avoid foods and beverages that can cause stains</li> <li>Micro-abrasion of the stains on the outer surface of teeth</li> <li>Bonding -tooth can be covered with a color-matched composite bonding material</li> <li>Veneers are color-matched thin ceramic shells that cover the outer surfaces of the teeth.</li> <li>Applying a bleaching agent to the enamel of the teeth.<br /> a. Vital bleaching over-the-counter whitening agents or in-office whitening procedures<br /> b. Unlike the well researched topic of health policy, there is a gap in the marketplace covering mental health policy and health care policymaking. This book fills that gap; it is a comparative analysis of the implementation of Assertive Community Treatment (ACT), an evidence-based practice employed in two states that promises to empower the well-being of individuals suffering from mental illness.</p> <p><em>Assertive Community Treatment</em> specifically examines the tension separating the notion of client recovery and evidence-based programs. Johnson challenges the assumption that practitioners should rely on evidence-based practices to close the gap between scientific knowledge and practice. She argues that in an era of managed care, this encourages state mental health administrators to adopt policies that are overly focused on outcomes. Programs that can measurethe outcomes of care provided, and evidence-based practices, have become central aspects of the quality care agenda.</p> <p>This study traces the role of policy entrepreneurs throughout the Assertive Community Treatment policymaking process. By differentiating mental health in general, qualitative research increases the chances of observing similarities and differences in outcomes. Johnson explains why the ACT model was adopted and implemented. She concludes that there is a clear monopoly by medical researchers and scientists within Assertive Community Treatment research, and as a result, too much emphasis is placed on the roles of policy entrepreneurs as the main innovators in the agenda and policy formulation stages. <p>Recently I have some patients who went on a tour of Malaysia coming back with very bad symptoms of motion sickness such as giddiness and vomiting.<br /> Any patient who has ever been sick to the stomach on a rocking boat or a bumpy airplane ride will know the discomfort of motion sickness.<br /> It does not produce long-term problems but it can make the life miserable, especially if the patient travels a lot.<br /> Women, older adults and children from 5 to 12 years old have motion sickness more than others.<br /> It is not frequent in children younger than 2 years.<br /> Motion sickness is occasionally called airsickness, seasickness, or carsickness.<br /> Motion sickness is a frequent problem in people traveling by car, train, airplanes, and particularly boats.<br /> Anyone can get it, but it is more frequent in children, pregnant women, and people taking certain medicines.<br /> Motion sickness can begin suddenly, with a queasy feeling and cold sweats.<br /> It can then result in dizziness and nausea and vomiting.<br /> The brain senses movement by getting signals from the inner ears, eyes, muscles, and joints.<br /> When it gets signals that do not match, the patient can get motion sickness.<br /> When the patient is reading on the phone while riding a bus, the eyes are focused on something that is not moving, but the inner ear senses motion.<br /> The inner ear balance mechanisms sense different signals to those that the eyes are seeing which then transmits to the brain mixed, confusing messages.<br /> This then produces the symptom of feeling sick (nausea).<br /> Other symptoms the patient may have are sweating, drooling saliva, headaches, feeling cold and going pale.<br /> Motion sickness can also be stimulated by anxiety or strong smells, such as food or petrol.<br /> Occasionally trying to read a book or a map can trigger motion sickness.<br /> In both children and adults, playing computer games can occasionally induce motion sickness.<br /> It is not known why some people develop motion sickness more than others.<br /> Symptoms can occur in cars, trains, planes and boats and on fairground rides, etc.<br /> Symptoms normally go when the journey is over; however, not always.<br /> In some people they persist a few hours, or even days, after the journey ends.<br /> The site that the patient sits can make a difference.<br /> These locations are best for preventing motion sickness:</p> <ol> <li>The front seat of a car,</li> <li>Forward cars of a train,</li> <li>Upper deck on a boat or</li> <li>Wing seats in a plane<br /> Looking out into the distance rather than trying to read or look at something in the vehicle can also help.<br /> Motion sickness can cause:</li> <li>A general feeling that the patient is ill.</li> <li>Nausea.</li> <li>Vomiting.</li> <li>Headache.</li> <li>Sweating.<br /> Symptoms will normally disappear soon after the motion ceases.<br /> Where there is a motion stimulus the diagnosis comes easily.<br /> In abnormal cases, central nervous causes of vertigo may need to be ruled out.<br /> A pregnancy test is likely to be the most useful additional lab study to exclude hyper-emesis gravidum.<br /> Treatment:<br /> There are several medicines present which can decrease or deter the symptoms of motion sickness.<br /> They work by interrupting with the nerve signals sent.<br /> Although they are best taken before the journey, they still may help even if the patient takes them after symptoms have begun.</li> <li>Scopolamine, which comes as a patch the patient put behind the ear.</li> <li>Medicines called antiemetics, which reduce nausea such as ondansetron (Zofran) and prochlorperazine (Compazine).</li> <li>Certain antihistamines, such as dimenhydrinate (Dramamine) and meclizine (Antivert, Bonine).<br /> Other methods:</li> <li>Eat a few dry soda crackers.</li> <li>Sip on clear, fizzy drinks such as ginger ale.</li> <li>Get some fresh air.</li> <li>Lie down or at least keep the head still</li> </ol> <p>TABLE OF CONTENT<br /> Introduction<br /> Chapter 1 Motion Sickness<br /> Chapter 2 Causes<br /> Chapter 3 Symptoms<br /> Chapter 4 Diagnosis<br /> Chapter 5 Treatment<br /> Chapter 6 Prognosis<br /> Chapter 7 Dizziness<br /> Chapter 8 Vertigo<br /> Epilogue</p>画面が切り替わりますので、しばらくお待ち下さい。 ※ご購入は、楽天kobo商品ページからお願いします。※切り替わらない場合は、こちら をクリックして下さい。 ※このページからは注文できません。
