＜p＞This book describes Popliteal Muscle Tear, Diagnosis and Treatment and Related Diseases which is seen in some of my patients in my Family Clinic＜/p＞ ＜p＞Last month I had a 50 year old man who came to consult me because of pain the right knee after he fell while climbing a stairway.＜/p＞ ＜p＞He had pain in his right knee together with swelling and a bruise just below the knee.＜/p＞ ＜p＞He had taken pain killers and placed ice on the right knee without improvement.＜/p＞ ＜p＞On examination he was found to be tender behind the right knee and there was limitation of right knee movement forward and flexing.＜/p＞ ＜p＞There was swelling of the knee.＜/p＞ ＜p＞The left knee was normal.＜/p＞ ＜p＞I decided to send him for an X-ray of the right knee which showed no evidence of fracture or dislocation of the bones in the knee joint.＜/p＞ ＜p＞A MRI was done and this showed the presence of a partial popliteal muscle tear.＜/p＞ ＜p＞The bones of the joint and other cartilages were normal.＜/p＞ ＜p＞He was sent to orthopedic surgeon for treatment.＜/p＞ ＜p＞Because the popliteal muscle tear was mild, he was told to rest his knee and to wear a brace to protect his knee.＜/p＞ ＜p＞Physiotherapy was provided in the form of knee muscle strengthening exercises.＜/p＞ ＜p＞He was also taught the proper method to climb up and down the stairs,＜/p＞ ＜p＞A tear in the popliteal tendon, also known as the popliteus tendon, can cause pain, instability, and difficulty with movements involving the knee joint.＜/p＞ ＜p＞The popliteus is a small muscle present at the posterior area of the knee joint almost traversing horizontally the back of the joint.＜/p＞ ＜p＞This small muscle is utilized for internal rotation of the tibia and for unlocking the knee joint when a person bends the knee from an entirely straight position.＜/p＞ ＜p＞Dynamic functions:＜/p＞ ＜p＞During early flexion of the knee, the popliteus muscle maintains the internal rotation of the tibia on the femur (unlocking of knee joint) and stops forward dislocation of the femur on the tibia.＜/p＞ ＜p＞Static functions＜/p＞ ＜p＞The popliteus muscle is an essential static stabilizer of the posterolateral corner of the knee and functions as a secondary restraint to posterior displacement of the tibia in PCL deficient knees.＜/p＞ ＜p＞Injury to the popliteus muscle is normally an excessive injury leading to higher tone or tightness in the muscle.＜/p＞ ＜p＞Poor biomechanics, faulty training and tense hamstrings are the frequent causes.＜/p＞ ＜p＞A fall or a tackle during sport activities where the knee is excessively straightened causes a tear of the popliteus muscle.＜/p＞ ＜p＞A sudden forceful impact or a direct blow to the back of the knee can lead to a popliteal tendon tear.＜/p＞ ＜p＞Repetitive activities or excessive stress on the tendon, such as in athletes involved in sports that require running, jumping, or pivoting, can result in a tear.＜/p＞ ＜p＞Chronic degeneration of the tendon due to aging, repetitive stress can weaken the tendon and increase the risk of tearing.＜/p＞ ＜p＞Frequent symptoms of a popliteus muscle tear may be:＜br /＞ Pain behind the knee＜br /＞ Swelling and tenderness＜br /＞ Weakness or instability in the affected knee joint＜/p＞ ＜p＞MRI can help in grading the injury and will make known related injuries such as lateral collateral ligament injury, anterior or posterior cruciate ligament injury and meniscal tears.＜/p＞ ＜p＞Minor tears may heal on their own with conservative treatment such as RICE therapy.＜/p＞ ＜p＞Physical therapy exercises to strengthen the knee and surrounding muscles aid in the recovery process.＜/p＞ ＜p＞More severe tears or complete ruptures may require surgical intervention.＜/p＞ ＜p＞TABLE OF CONTENT＜br /＞ Introduction＜br /＞ Chapter 1 Popliteal Muscle Tear＜br /＞ Chapter 2 Cause＜br /＞ Chapter 3 Symptoms＜br /＞ Chapter 4 Diagnosis＜br /＞ Chapter 5 Treatment＜br /＞ Chapter 6 Prognosis＜br /＞ Chapter 7 Knee Exercises＜br /＞ Chapter 8 Popliteal (Baker) Cyst＜br /＞ Epilogue＜/p＞画面が切り替わりますので、しばらくお待ち下さい。 ※ご購入は、楽天kobo商品ページからお願いします。※切り替わらない場合は、こちら をクリックして下さい。 ※このページからは注文できません。

＜p＞Due to the ongoing recent expansion of public interest issues worldwide, the state's right to regulate has been recaptured as a prominent concept in international investment law. The fair and equitable treatment (FET) standard provision in the text of an international investment agreement (IIA) has become a detailed clause clarifying the specific obligations of a state towards an investor under the FET standard. However, striking the right balance between the interests of host states and investors in these new treaty formulations has proved to be challenging. This book greatly clarifies the field by offering the in-depth analysis of the application of the state's right to regulate in relation to FET standard provisions in IIAs and to decisions by arbitral tribunals in FET cases. Recognising that the role of tribunals is to balance the state's public interests and the interests of the investor when interpreting and applying the FET standard, the author pursues such seminal issues and topics as the following: the legitimacy of the objective of the state's measure; obligations and responsibilities of investors towards a host state; the nature and impact of a change to a national regulatory framework; special economic and sociopolitical circumstances in a host state; and due diligence and risk assessment as a condition for the protection of an investor's legitimate expectations. Multiple IIAs concluded by the OECD Member States, as well by Russia and China between the developing countries, and the prominent investment law cases on the FET standard are examined in detail. The analysis pays particular attention to how investment jurisprudence in FET cases has been reflected in such new IIAs as the Comprehensive Economic and Trade Agreement between the European Union (EU) and Canada (CETA), the EU-Vietnam FTA and the EU-Singapore FTA. These case studies demonstrate the evolution of the IIAs' FET standard provisions and how they balance the application of the FET standard and the state's right to regulate. Suggestions are provided for drafting formulations of the FET standard that can contribute to achieving such a balance. ＜p＞This book describes Levator Ani Syndrome, Diagnosis and Treatment and Related Diseases＜/p＞ ＜p＞Levator ani Syndrome is an episodic rectal pain produced by the spasms of the rectal muscle, called the levator ani.＜/p＞ ＜p＞The levator ani is a thin, broad muscle which is located on the side of the pelvis.＜/p＞ ＜p＞This syndrome comprises:＜/p＞ ＜ol＞ ＜li＞Pain,＜/li＞ ＜li＞Gluteal discomfort and＜/li＞ ＜li＞Distress in the rectal area,＜/li＞ ＜/ol＞ ＜p＞It spreads to the region of the sacrum and coccyx.＜/p＞ ＜p＞The pain and discomfort produced by this disease is often exacerbated by sitting.＜/p＞ ＜p＞Levator ani syndrome is a pelvic floor dysfunction where the muscles are not relaxed.＜/p＞ ＜p＞It indicates that the pelvic floor muscles are too tight.＜/p＞ ＜p＞The pelvic floor strengthens the rectum, bladder, and urethra.＜/p＞ ＜p＞In women, levator ani muscle also has supported the uterus and vagina.＜/p＞ ＜p＞Levator ani syndrome is more frequent in women.＜/p＞ ＜p＞Its main symptom is constant or frequent dull pain in the rectum caused from a spasm in the levator ani muscle, which is near the anus.＜/p＞ ＜p＞Generally speaking, pelvic pain as a whole is considered a neuropathic disorder indicating the pain is caused by nerve damage which causes pain in the associated area.＜/p＞ ＜p＞Levator ani syndrome, on the other hand, is a combination of neuropathic and muscular pain which makes it much harder to tease out from other types of pain.＜/p＞ ＜p＞It is not obvious if the muscle dysfunction is what led to nerve damage or the other way around.＜/p＞ ＜p＞Either way, one cannot effectively treat levator ani syndrome without treating both pathologies.＜/p＞ ＜p＞The precise cause of levator ani syndrome is not known.＜/p＞ ＜p＞It may be related to any of these:＜/p＞ ＜ol＞ ＜li＞Not urinating or passing stool when needed＜/li＞ ＜li＞Vaginal shrinking (atrophy) or pain in the vulva (vulvodynia)＜/li＞ ＜li＞Continuing sexual intercourse even when it is painful＜/li＞ ＜li＞Injury to the pelvic floor from surgery or trauma, such as sexual abuse＜/li＞ ＜li＞Having another type of chronic pelvic pain:＜br /＞ a. Irritable bowel syndrome,＜br /＞ b. Endometriosis＜br /＞ c.
