＜p＞This book describes Dizziness, Diagnosis and Treatment and Related Diseases＜/p＞ ＜p＞This week I had a patient who has the sensation of dizziness which occurred after an operation on his sinuses to remove bloody pus.＜br /＞ He felt the dizziness worse when moving from the lying to the upright position.＜br /＞ Whether it was the after effect of the anesthesia or the operation itself, he was certainly paler than usual and feeling tired.＜br /＞ He was told to return to his ear-nose and throat surgeon for a review including X-ray of his sinuses and a full blood count.＜br /＞ His blood tests showed evidence of mild anemia (low hemoglobin) and low red blood cells count possibly from the loss of blood from the sinusitis and during the operation.＜br /＞ He was told to take Iron with folic acid and multivitamins to build up his blood, stemetil (prochlorperazine) for his dizziness and told to have adequate rest and eat more red meat.＜br /＞ In the meantime his MRI showed evidence of mucosal thickening of the sinus walls and evidence of mild infection.＜br /＞ An antibiotic gel was inserted into his sinuses to try to clear the sinus infection.＜br /＞ In spite of all these measures he still felt dizzy but not as bad as previously.＜br /＞ Since there was no evidence of malignancy, I tried to reassure him and not to worry too much as stress and worry by itself can cause the dizziness.＜/p＞ ＜p＞Dizziness has been used to depict different sensations.＜br /＞ True dizziness is a feeling of light-headedness, unsteadiness or simply feeling faint.＜br /＞ It involves the sensory organs, specifically the eyes and ears, so it can occasionally cause fainting.＜br /＞ Most causes of dizziness are not severe, and they either rapidly get better on their own or are easy to treat.＜br /＞ Dizziness is not a medical disorder, but rather a symptom of different disorders.＜br /＞ The symptom of "dizziness" needs to be separated from vertigo, which is a spinning sensation feeling that the room is moving, or that the person is moving when they are not (or even a floating sensation).＜br /＞ Vertigo is a feeling that the patient is spinning or moving, or that the world is spinning around the patient.＜br /＞ Balance difficulty is also a separate disorder where a person feels they are about to fall, has difficulty keeping balanced, or may actually fall.＜br /＞ It may also feel like motion sickness or as if the patient is leaning to one side.＜br /＞ Vertigo and disequilibrium may cause a feeling of dizziness, but those 2 terms depict different symptoms.＜br /＞ Disequilibrium is a loss of balance or equilibrium.＜br /＞ True dizziness is the sensation of lightheadedness or near fainting.＜/p＞ ＜p＞Occasional dizziness is not something to worry about.＜br /＞ The patient should call the doctor immediately if the patient is feeling repeated episodes of dizziness for no apparent reason or for a prolonged period.＜br /＞ Lightheadedness happens when the brain does not get enough blood.＜br /＞ This may happen if:＜br /＞ 1.The patient has a sudden drop in blood pressure.＜br /＞ 2.The body does not have enough water (is dehydrated) due to vomiting, diarrhea, fever, and other disorders.＜br /＞ 3.The patient gets up too quickly after sitting or lying down (this is more frequent in older people).＜br /＞ 4.Lightheadedness may also happen if the patient has the flu, low blood sugar, a cold, or allergies.＜br /＞ More serious disorders that can lead to light-headedness are:＜br /＞ 1.Heart disorders, such as a heart attack or abnormal heart beat＜br /＞ 2.Stroke＜br /＞ 3.Bleeding inside the body＜br /＞ 4.Shock (extreme drop in blood pressure)＜br /＞ If any of these serious disorders are present, the patient will normally also have symptoms like chest pain, a feeling of a racing heart, loss of speech, change in vision, or other symptoms.＜br /＞ Vertigo may be due to:＜br /＞ 1.Benign positional vertigo, a spinning feeling＜br /＞ 2.Labyrinthitis＜br /＞ 3.Meniere Disease＜/p＞ ＜p＞TABLE OF CONTENT＜br /＞ Introduction＜br /＞ Chapter 1 Dizziness＜br /＞ Chapter 2 Causes＜br /＞ Chapter 3 Symptoms＜br /＞ Chapter 4 Diagnosis＜br /＞ Chapter 5 Treatment＜br /＞ Chapter 6 Prognosis＜br /＞ Chapter 7 Meniere’s Disease＜br /＞ Chapter 8 Benign Paroxysmal Positional Vertigo＜br /＞ Epilogue＜/p＞画面が切り替わりますので、しばらくお待ち下さい。 ※ご購入は、楽天kobo商品ページからお願いします。※切り替わらない場合は、こちら をクリックして下さい。 ※このページからは注文できません。

＜p＞Angiogenesis (formation of new vessels from pre-existing ones) is a crucial early event in the process of tumor development. New vessels supply the tumor with nutrients that are needed for further local growth and enable distant metastases (Folkman 1995). Judah Folkman (1971) highlighted the potential therapeutic imp- cations of tumor angiogenesis. He hypothesized that if tumor angiogenesis is inhibited, then tumor growth and metastasis will be impaired greatly or even impossible. The subsequent quest for endogenous and exogenous inhibitors of angiogenesis has yielded a variety of promising therapeutic agents that block one or more angiogenic pathways, a few of which have been approved by the FDA (e. g. , bevacizumab, sorafenib, sunitinib) for use as single agents or in combination with chemotherapy in specific populations of cancer patients (Sessa et al. 2008). There has also been a dramatic expansion in the exploration of novel anti-angiogenic agents pre-clinically and in clinical trials (Ferrara 2002). Some of the most promising data comes from the development of agents that inhibit one of the key growth factors involved in tumor angiogenesis ? vascular endothelial growth factor (VEGF) (Ferrara et al. 2003). Bevacizumab is a monoclonal antibody against VEGF that was the first an- angiogenic agent that improved significantly the overall survival of patients with colorectal and non-squamous non-small cell lung cancer (Ferrara et al. 2005). ＜p＞Fosfomycin is an antimicrobial substance which is available since several years. In earlier clinical studies an oral preparation with poor absorption in a low dosage resulted in inadequate therapeutic results. The presently used parenteral administration in a dosage range of up to 250 mg/kg BW however allowed us to appreciate several unique properties of this drug which make it valuable in the treatment of serious bacterial infections. Due to its small molecular weight and complete lack of protein binding a favorable penetration in tissues and deep compartments (e.g., meninges, bone, heart valves) was observed. In addition fosfomycin shows a remarkably low toxicity and even protects from the nephrotoxigenic effects of aminoglycosides and other drugs. Earlier problems in antimicrobial susceptibility testing have been solved by addition of glucose 6 phosphate into the growth medium. Fosfomycin, the first representative of a completely new class of antibiotics does not show cross-resistance with other antibiotics; therefore pathogens of nosocomial infections, particularly methicillin- und gentamicin-resistant staphylococci are mostly susceptible to this antibiotic. Clinical investigations show a favorable therapeutic outcome in severe nosocomial infections despite of previous therapeutic approaches with various other potent antibiotics. ＜p＞The data about different aneurysm locations given in the present work clearly demonstrate that the quality and the severity of the neuropsycho logical impairments after subarachnoid hemorrhage are in particular dependent on the anatomical location and extent of the bleeding. In addition, the sometimes inevitable temporary clipping of perforating vessels seems to play a significant role with respect to later neuropsychological disturbances. Since modern aneurysm surgery is performed in the acute phase shortly after the hemorrhage, the findings reported in the present work are, according to my opinion, of particular relevance. In the light of the present intensive discussion about the indication for microneurosurgical clipping or neuroradiological interventional coiling of intracranial aneurysms, the results given here in by B. O. Hutter can be regarded as an argument for the surgical intervention, in particular because the extravasated blood can only be cleared by surgery. Therefore, this book may be an inspiration for the neurosurgical reader for a closer collaboration with psychologically trained scientists.
